The impact on services and the pressures faced by our front-line health and care services has been evident, more so during the pandemic. With the peak of winter pressures still to come, Forum Central is working with partners across the health and care sector to alleviate some of the pressures, and ultimately improve the experience of those leaving hospital.

A new programme of funding is being developed, managed by Forum Central Partner Leeds Older People’s Forum, to enable a cross sector partnership approach to supporting our citizens. The programme will adopt a test and learn approach used by Leeds Older People’s Forum so successfully in the Time to Shine programme and will run up to 31 March 2023.

The work delivered by our colleagues in the Third Sector to support Leeds citizens has been life changing for so many people. We want to bring together this valuable expertise and link in with hospital and neighbourhood care teams to offer wraparound welfare throughout a patient’s journey and avoid delayed discharges and readmissions.

You are invited to join us online on Friday 14 January 2022 at a practical workshop to shape the programme. Together, we’ll look at what support and services already exist, what works well and what we might need.
