Age UK Leeds is supporting Leeds Teachings Hospital Trust with hospital discharges and providing Winter Warm Packages to support vulnerable older people.

One of the initiatives they have introduced is the Winter Warm scheme, where we can provide winter support packages to support vulnerable older people.

Winter Warm Packages:

  • Warm Food
  • Warm Clothing
  • Blankets and Bedding
  • Small Household Goods
  • Prescription Collection
  • Food Shopping
  • General Welfare Checks

They are also scaling up their existing Hospital to Home Service, to target more people most at risk of admission to hospital and who need extra support when they are discharged, such as older people living alone.

NHS England is funding voluntary sector organisations including Age UK to provide extra help over the busy winter months

This includes upscaled Hospital Discharge Support from St James’s Hospital provided by Age UK Leeds Hospital to Home Team
