The SWIFT Community Engagement Project is an exciting and innovative project working with people living within the Armley & Wortley district of Leeds 12.
The project ambition is to develop inclusive services that reflect the aspirations and needs of people aged 50+.
The SWIFt community engagement project will support people who are living with frailty or with complex issues who are often socially isolated, to improve their wellbeing and independence.
The SWIFt Community Engagement Worker will respond to the needs and aspirations of the older person, will carefully listen to them and try to respond appropriately, by providing a holistic assessment of what matters to the older person.
This is a role where you can talk to, meet and be responsive to people’s needs and make a difference by supporting individuals to engage within their local community and pursuing individual’s personal goals, in line with their choices and preferences and in way which are socially inclusive and enabling people to live independently and without fear of crime, isolation and Poverty.
