Hours: Full time (but may consider part-time)

Length of Contract: Fixed term until at least December 2021

The Leeds Frailty Programme Board is looking to appoint a programme manager on a fixed term contract. The post will be hosted by the System Integration Team on behalf of the Leeds Health and Care System.

Expressions of interest are being sought from employees of the CCG and Leeds Health and Care System partner organisations for this exciting opportunity, which will be at the forefront of transformation within Leeds.

The CCG has been working with partners across the Leeds Health and Care System to progress commissioning for outcomes, applying population health management techniques, since 2017.

The first population group with which to test out new ways of working is people living with frailty.

This post comprises a challenging portfolio of core development and specialist policy. It is a significant contribution towards the overall CCG and health and care system strategic aim of commissioning for outcomes, population health management and provider integration, and redressing inequalities in health.

An overall aim of the programme is also to improve outcomes for people living with frailty through focusing on what matters most to people.

As well as robust programme management skills this post requires a strong inclusive partnership approach and system leadership skills which are crucial both internally and externally including political astuteness, relationship building, engaging, influencing and negotiating.

The ideal candidate would be comfortable with a working environment and strategic context which is often uncertain, complex and ambiguous and be able to recognise the positive opportunities of working within such an environment.
