Forum Central has produced a glossary explaining the meaning of commonly used acronyms and abbreviations of key terms relating to health and care locally and nationally.

Acronym	Meaning	Definition/useful information
ABCD	Asset Based Community Development	A methodology for the sustainable development of communities based on their strengths and potentials.
ALaDDIN	Autism and Learning Disability Digital Inclusion Network	A group of 20 third-sector organisations Leeds working to improve digital inclusion and participation for people with autism or learning disabilities.
ASC	Adult Social Care
ATR	Adversity Trauma Resilience
BAME / BME	Black, Asian and Minority Ethnic / Black and Minority Ethnic	This is an outdated term not in current use but may be on older documents. The criticism is that it is a large generalisation which emphasises certain ethnic minority groups and excludes others. Best practise is to be specific about the population you are referring to or if generalising, use the terms, culturally diverse communities or Global Majority.
BLB	Better Lives Board	Brings together Leeds City Council’s Adults and Health Directorate and people with lived experience of Health and Care support/services.
BLC	Big Leeds Chat	The Big Leeds Chat is a way of listening to people in Leeds. It brings together the most senior decision makers in health and care, working together as one health and care team to talk to members of the public, of all ages and backgrounds, about what matters to them.
BLM	Black Lives Matter	A world-wide movement that aims to highlight racism, discrimination, and racial inequality experienced by black people and to promote anti-racism.
CEV	Clinically Extremely Vulnerable	A cohort of people who are at high risk of severe illness from coronavirus and have been advised to shield at certain points of the pandemic to reduce their risk of catching the virus.
CIC	Community Interest Company	A Third Sector organisation which is a social enterprise that uses profits and assets for the public good. CICs are intended to be easy to set up, with all the flexibility and certainty of the company form, but with some special features to ensure they are working for the benefit of the community.
CLASP	Citywide Leeds Anti Stigma Partnership
CBMHR	Community Based Mental Health Reviews
CMHT	Community Mental Health Transformation
CMHT	Community Mental Health Teams	Teams that support or treat people with mental illness in a community setting.
CoI	Communities of Interest	Groups of people who share an identity, for example people with a learning disability, or those who share an experience, for example the homeless community.
COMHAD	Co occuring Mental health/Drug And Alcohol Misuse	Previously known as Dual Diagnosis.
CQC	Care Quality Commission	The independent regulator of all health and social care services in England.
CYP	Children and Young People
DAWN	Disability And Wellbeing Network	A group of staff at Leeds City Council promoting all levels of ability, not disability.
DGL	Doing Good Leeds	A website which acts as a Hub for the for the voluntary, community and social enterprise sector (third sector) in Leeds, powered by Voluntary Action Leeds.
GP Confed	Leeds GP Confederation	This is a ‘not for profit social enterprise,’ working to improve the health of the people of Leeds by strengthening and sustaining primary care.
FC	Forum Central	The collective voice for the health and social care third sector in Leeds.
H&C	Health and Care
H&SC	Health and Social Care
HPoC	Harnessing the Power of Communities	West Yorkshire & Harrogate programme to establish relationships with communities built around good work on the co-production of services and care.
HW	Healthwatch	Independent watchdog ensuring people’s voices are at the heart of shaping health and care services in Leeds.
HWB	Health and Wellbeing Board	The place-based forum which brings together leaders from the local health and care system. Responsible for producing a joint strategic needs assessment and a joint health and wellbeing strategy.
IAPT	Increasing Access to Psychological Therapies	A national initiative, in Leeds now under Leeds Mental Wellbeing Service (see LMWMS below).
ICB	Integrated Care Board	The statutory organisation responsible for leading NHS integration at West Yorkshire level.Develops a plan to meet the health needs of the population and secures the provision of health services. Directly accountable for NHS spend and performance within the system.
ICP	Integrated Care Pathways	Multidisciplinary outlines of anticipated care, placed in an appropriate timeframe, to help patients with a specific condition.
ICP	Integrated Care Partnership	An alliance of NHS, local authority and Third Sector providers that work together to deliver care collaboratively. Now known as a Place Based Partnership.
ICS	Integrated Care System	Partnership that brings together providers and commissioners of NHS services across a geographical area with local authorities and other local partners, to collectively plan and integrate care to meet the needs of their population. Also referred to as WYH/WYHP regionally.
IG	Information Governance	The overall strategy for information at an organisation.
KLOE	Key Line of Enquiry	Used by the Care Quality Commission (CQC) to guide and direct their inspections of care services.
LCC	Leeds City Council	The local authority of the City of Leeds, West Yorkshire.
LCF	Leeds Community Foundation	An organisation that supports thousands of charities and voluntary groups across the city by distributing grants and sharing advice.
LCH	Leeds Community Healthcare	NHS Trust which provides a range of community-based health services across the Leeds area.
LCP	Local Care Partnership	Leeds’ model of joined-up working formed around Primary Care Networks to improve health and care delivery to local people, working in and with local communities. We have 19 across Leeds.
LD	Learning Disability
LeDeR	Learning Disability Mortality Review	National programme aimed at making improvements to the lives of people with learning disabilities.
LFP	Leeds Frailty Partnership
LGBT, LGBTQ+, LGBTQIA+	Lesbian Gay Bisexual Trans Queer Questioning Intersex Asexual ‘plus’	The ‘plus’ symbol indicates that there are many other identities welcomed within the community.
LGI	Leeds General Infirmary	One of the main hospitals in Leeds.
LHCP	Leeds Health and Care Partnership	The name of the place-based partnership in Leeds – the ICB at place level.
LIP	Leeds Involving People	Leeds Organisation set up to support service user and carer involvement in planning community care services.
LL	Linking Leeds	Leeds’ Social Prescribing Service.
LMWBS	Leeds Mental Wellbeing Service	A joint service between NHS partners and third sector organisations providing support and psychological therapies for mental health problems. Formerly known as IAPT (see above).
LOOP	Leeds Open Online Platform	A proposed solution for a single source of accurate, up to date and trusted information about health, care and wellbeing, shared with directories, websites and applications across Leeds.
LOPF	Leeds Older People’s Forum	The registered charity advocating on behalf of older people in Leeds, they also bring together a network of voluntary sector organisations and individuals, including the Neighbourhood Network Schemes. Part of the Forum Central partnership.
LSSPG	Leeds Strategic Suicide Prevention Group
LTC	Long-Term Conditions
LTHT	Leeds Teaching Hospitals Trust	NHS Trust which provides healthcare and specialist services for people from Leeds, Yorkshire and the Humber and beyond; spread over seven hospitals and medical facilities including St James’ and LGI.
LWL	Live Well Leeds	Community Based Mental Health Service.
LYPFT	Leeds and York Partnership Foundation Trust	NHS Trust which provides secondary care specialising in mental health and learning disabilities services in Leeds.
MAP	Migrant Access Project	A citizen-led initiative to build relationships between newly arrived migrants and service providers in Leeds.
MDT	Multi Disciplinary Team	A group of trained staff from different areas of healthcare, who meet together every week, to discuss the care of individual patients.
MH	Mental Health
MHPB	Mental Health Partnership Board
NHS	National Health Service
NHSE	National Health Service England
NICE	National Institute for Health and Care Excellence	Provides national guidance and advice to improve health and social care.
NIHR	National Institute for Health Research	A United Kingdom government agency which funds research into health and care.
NN, NNS	Neighbourhood Networks, Neighbourhood Network Scheme	The Leeds model of voluntary sector organisations supporting older people covering the whole city; sometimes referred to as good neighbours schemes.
OP	Older People
PBP	Place-based Partnership	An ‘umbrella’ term for the local partnerships which bring together health, care, voluntary community social enterprise (VCSE) organisations in each of our places) to improve health and wellbeing. Until recently, these were known as local integrated care partnerships (ICPs).
PCN	Primary Care Network	A group of local health services, including GPs, which work together in an area to treat populations of around 30-50,000. The networks provide the structure and funding for services to be developed locally, in response to the needs of the patients they serve.
PCWBS	Primary Care Wellbeing Service
PLT (formerly PEG	Partnership Leadership Team (of the Leeds Health and Care Partnership)	Leadership group comprised of representatives from the Leeds Health and Care Partnership/ICB in Leeds. Formerly Partnership Executive Group.
PH / PHE	Public Health England
PH LCC	Public Health Leeds City Council
PHM	Population Health Management	A patient centric, data driven approach to improving the health of a population.
PHRC	Public Health Resource Centre	Offers support to anyone with a responsibility for or professional interest in public health or promoting health and wellbeing in Leeds.
PID	Project Initiation Document
PLWD	People Living with Dementia
PPI	Patient Participation and Involvement
PSI	Physical and Sensory Impairment
PVG	People’s Voices Group	Set up by the Leeds Health and Wellbeing Board to help improve engagement across the City with health and care organisations, coordinated by Healthwatch Leeds.
SEYH	Social Enterprise Yorkshire and the Humber	The membership organisation of social enterprises or associates with an interest in the sector for Yorkshire and the Humber.
SRO	Senior Responsible Officer
THIG	Tackling Health Inequalities Group	Group formed during the pandemic with representation from key organisations across the city, facilitated by Leeds City Council, to tackle health inequalities through a whole systems and place based approach.
TNLCF	The National Lottery Community Fund
TOR	Terms of Reference	Define the purpose and structures of a project, committee, meeting, negotiation, or any similar collection of people who have agreed to work together to accomplish a shared goal
TS	Third Sector	A term used to refer to charities, social enterprises and voluntary groups. Also known as voluntary or not for profit sector
TSL	Third Sector Leeds	An alliance that promotes, represents and advocates for the Third Sector in Leeds
TSL	Targeted Services Leader (in Cluster Services context)	Clusters are groups of professionals (schools, social workers, housing etc.) who coordinate services for families in their local areas. Each cluster has a Targeted Services Leader, who makes sure clusters work together and young people and families can access their services effectively
TSO	Third Sector Organisations	Organisations working in the Third Sector (see above)
TTS	Time To Shine	The National Lottery Community Fund programme aiming to reduce social isolation and loneliness amongst older people in Leeds, led by Leeds Older People’s Forum
VAL	Voluntary Action Leeds	The Leeds-wide infrastructure charity that supports 3k different not for profit organisations. VAL supports communities by helping people and organisations that do good in Leeds
VCSE, VCE	Voluntary, Community and Social Enterprise Sector (sector)	Also referred to as the Third Sector, not for profit sector, NGOs (non-governmental organisations).
WYHCP	West Yorkshire Health and Care Partnership	A partnership of organisations and NHS Trusts working together to plan and deliver health and care services across the region.
