Recent conversations across Leeds health and care have continued to highlight the growing focus on neighbourhood health, partnership working and more joined-up services across the city.

A new update shared by the Leeds Health Partnerships Team outlines ongoing work around the Leeds Provider Partnership (LPP), including the development of governance arrangements, collaborative working models and preparations for a “shadow year” during 2026/27 ahead of more formal arrangements expected from April 2027 onwards.

The update reflects a wider national shift towards neighbourhood health, prevention and more integrated care delivered closer to communities. Much of this direction of travel has already featured across recent Forum Central discussions, blogs and Reps & Reference Group meetings.

Over recent months, several key themes have emerged consistently:

  • moving more care and support into neighbourhoods and communities
  • strengthening partnership working across NHS, local authority and VCSE organisations
  • reducing duplication and fragmentation across services
  • improving prevention and earlier intervention
  • creating more joined-up pathways around people and communities rather than organisational boundaries

This work connects closely with the government’s wider Neighbourhood Health Framework, which outlines ambitions to move services from hospital to community, from sickness to prevention, and towards more integrated local delivery models.
