The Leeds HomeFirst Programme is now known as the Neighbourhood Health – Adults Programme, bringing its work under the wider Neighbourhood Health approach across Leeds.

The new name reflects how existing work on intermediate care, prevention and neighbourhood-based support is evolving as part of a joined-up health and care system. While the name has changed, the ambition remains the same: helping people live well and independently at home through closer collaboration across health, care and community partners.

Building on a successful programme

Since it began, the HomeFirst Programme has brought organisations across the Leeds Health and Care Partnership together to improve intermediate care, supporting people to recover at home, reducing unnecessary hospital stays and improving outcomes for people, carers and staff.

As the programme developed, its focus expanded beyond intermediate care to include prevention and neighbourhood proactive care. This work will now continue as part of the Neighbourhood Health – Adults Programme, aligning with Leeds’ long-term vision for more connected, preventative and person-centred care.

Importantly, this is not a new programme. It is a continuation of the work already taking place across the partnership, bringing existing programmes together under a single identity.

What the programme includes

The Neighbourhood Health – Adults Programme will continue to deliver work across both proactive and reactive care.

Proactive care

Current priorities include:

  • Scaling proactive care across every Primary Care Network (PCN) and Local Care Partnership (LCP) in Leeds.
  • Developing tiered models of care for people living with frailty and diabetes.
  • Improving mental health pathways.
  • Evaluating the impact of proactive care.

Reactive care

The programme also continues work to improve urgent and intermediate care through:

  • Community pathways, including Urgent Community Response developments.
  • Improvements to intermediate care and hospital discharge.
  • Stroke pathway development.
  • A 24/7 end of life support line.
  • Continued improvements to mental health pathways.

Supporting neighbourhood health in Leeds

Neighbourhood Health is a city-wide approach that brings together NHS organisations, Leeds City Council, the VCSE sector and other partners to organise care around people and communities rather than individual organisations.

By strengthening prevention, improving joined-up working and providing more support closer to home, the partnership aims to improve health outcomes, reduce health inequalities and help more people remain independent for longer.
