Colleagues across Leeds are being asked to take part in the latest Neighbourhood Proactive Care Staff Survey.

The survey is open to staff working across the system and aims to measure how proactive care is being embedded across partners and neighbourhood teams.

Who should complete it?

The survey is open to anyone working across community and neighbourhood health, including those involved in:

  • Clinical or non-clinical MDTs

  • Frailty or falls hubs

  • Mii Care

  • Advanced respiratory

  • Long-term conditions

  • The national neighbourhood health implementation programme

You are also encouraged to respond if you work with people in the community or in their own homes, even if you do not feel directly involved in proactive care.

The intention is to hear from the whole system.

What’s changed?

If you completed the baseline survey last summer, thank you.

Feedback was taken on board and this version is significantly shorter. It uses mainly tick-box responses and should take only a few minutes to complete, with one optional free-text box for additional comments.

Why it matters

The survey is designed to measure change over time as proactive care becomes embedded across partners.

Understanding:

  • What is working well

  • Where improvements are needed

  • How this way of working is landing across services

…will help shape the future development of neighbourhood and proactive care models.
