The NHS West Yorkshire Integrated Care Board is continuing its monthly Integrated Neighbourhood Health (INH) webinar series, bringing together partners from across West Yorkshire to share learning, experience and practical insight.
The fourth session in the series, Digital Integration: Shared Care Planning, will be led by Abraham Bwalya and Pam Bhupal and will focus on how shared care plans can support better coordination, safety and collaboration across health and care systems.
About this session
Shared Care Planning is increasingly recognised as a key enabler of joined-up, neighbourhood-based care. This session will explore how shared care plans are being used in practice to improve communication between services, reduce duplication, and support people to receive more coordinated care.
The webinar will include examples from areas already leading in this space, alongside honest discussion of the challenges teams have faced and how they are addressing them. Participants will hear both what has worked well and what has been more difficult, helping to ground the learning in real-world experience rather than theory alone.
The session is designed to help attendees reflect on how shared care planning might be embedded within their own organisation, neighbourhood or partnership.
