ABA Leeds in partnership with Leeds Older People’s Forum have launched the ENHANCE programme. They are currently accepting referrals from anyone leaving the hospital and needing support services. This programme is available to anyone aged 18+ and predominantly from the BAME communities
What services do they provide?
- Making meals and refreshments
- Ensuring they have enough provisions, topping up pre-payment metres, utility bills have been paid and a safe environment
- Prompting to take medication and joint visits to the nurse / GP
- Prompting to undertake exercise / activities
- Provide regular contact and befriending
- Linking into community activities and social connections
- Assisting with IT / digital use
- Ensuring adequate equipment to enable safe movement around the house and falls prevention assessment
- Referring patients with dementia to C&R dedicated dementia support worker
