The Harnessing the Power of Communities (HPoC) Small Grants Final Report, prepared in November 2023, highlights the impact of this unique fund. The grants programme was delivered in partnership by Forum Central, Leeds Community Foundation, the West Yorkshire Integrated Care Board and the West Yorkshire Health and Care Partnership.

A wide range of projects and communities have benefitted from these grants, which were aiming to:

  • Develop, strengthen and sustain health focused Third Sector community activity in Leeds.
  • Improve the health and wellbeing of people in communities identified as the most disadvantaged in the city.
  • Build the capacity of the Third Sector as an integral part of the health and care system.

The report summarises final outcomes and learning from the 31 Small Grants awarded to a variety of Community Organisations for 1 or 2 years of activity.  These were funded through the 2020 and 2021 rounds of the programme, and information about additional grants awarded in 2021-22 is also included.

Over 7000 people were reached or supported by the programme.
