Leeds Hospitals Charity has opened the next round of Innovation in Healthcare Delivery Grants, inviting applications for projects that improve how people move through health and care services. The fund supports practical ideas that make patient pathways smoother, reduce avoidable delays and strengthen joint working across the Leeds system.

The grants are aimed at staff within Leeds Teaching Hospitals NHS Trust (LTHT) and partner organisations who are developing innovative, evidence-informed approaches to improving access, flow or discharge. This includes work with VCSE organisations, social care, community health teams and other partners involved in patient support.

Why this fund matters

Improving flow through services is a key priority across health and care in Leeds. Many people still experience delays that affect their wellbeing, recovery and independence. VCSE organisations often play a central role in supporting smoother transitions by offering early help, prevention, and community-based support. This funding helps strengthen those relationships and enables teams to test solutions that would otherwise be difficult to resource.

What the fund will support

Projects can focus on:

  • Reducing discharge delays through better coordination or community support

  • Improving patient access to timely appointments, treatment or advice

  • Testing new technology, digital tools or process changes

  • Supporting people to avoid hospital attendance through targeted early help

  • Strengthening partnership models between LTHT teams and VCSE organisations

  • Piloting new approaches that can be scaled across the Trust or the wider system

Both small-scale tests and larger multi-team collaborations are encouraged, as long as they demonstrate clear benefit to patients.
