In total, we have been given £150,000 to fund the overall project. An initial phase was launched in Inner South Leeds in September 2025.

• Funding will be given based on a tiered approach:
o Up to £1k – no set minimum
o Up to £3k – minimum of 75 blood pressure checks
o Up to £5k – minimum of 100 blood pressure checks

Funding will be given to organisations from West Yorkshire ICB as a grant agreement and will be non-recurrent funding.

Open to community and third sector organisations in West Leeds, Woodsley & Holt Park, and Morley & District LCPs

Background information about the fund

Grant information and background about the fund can be found through their information session, which was recorded: Community Blood Pressure Grants Online Information Session | YouTube

Awareness Raising and Prevention

  • Raise awareness of the important role blood pressure plays in our health.
  • Help communities better understand risks, signs and symptoms of high blood pressure.
  • To improve access to blood pressure reading equipment locally and normalise regular BP reading.
  • Help communities to ‘know their numbers’.

Making Every Contact Count

  • BP checks create a natural opportunity to start conversations about health and lifestyle.
  • Staff can signpost individuals to local support services and resources
  • Trust and familiarity with community organisations encourages people to engage with their health
  • Social prescribing can encourage conversations around further support regarding the wider determinants of health

Improving Healthcare Outcomes

  • Early detection of hypertension reduces the risk of stroke, heart attack, and kidney disease.
  • Increased awareness and self-management of health conditions leads to better long-term control (e.g. diet, exercise, medication adherence).
  • Better engagement with prevention services, including NHS Health Checks and lifestyle programs.
