Local Care Partnerships (LCPs) are the model of joined-up working that delivers local care for local people, working in and with local communities.

They bring together partners across health, care and the VCSE sector to strengthen neighbourhood working, improve coordination and ensure services respond to local need.

If you work within the Leeds area and would like to better understand how LCPs operate — and how your organisation can connect — you are invited to attend an informal online introduction session.
