The Healthy Leeds Plan is our partnership plan which describes the health and care contribution to achieving the Leeds Health and Wellbeing Strategy.

The plan has recently been refreshed and provides two clear goals for us, as a partnership, to deliver with a clear focus on our most deprived communities. The plan is built and designed around our population health approach and the infrastructure we have embedded within Leeds. It is designed to put the diverse needs of our population at the heart of everything we do and move decision making closer to the people using our services.

A data led approach has been applied to the development of our plan through the Leeds Data Model. The Leeds Data Model is our pseudonymised, person level, linked dataset that brings together data from a range of partners delivering health and care services within Leeds. The model enables us to identify specific cohorts through our population segmentation model to support planning and prioritisation.

At this lunch and learn session will provide an overview of our plan and approach but most importantly get you to become data curious, providing you with access and support to the data tools that have been developed for use across the partnership.
