Managed by Leeds Older People’s Forum on behalf of Forum Central, year 2 of Enhance wrapped up in May, and year 3 is well underway.

Dr Sarah Alden, an Independent Research, Evaluation and Learning Consultant, has produced a summary of the findings for the second year of this valuable programme.

Enhance supports safe and sustainable discharge from hospital and Neighbourhood Teams into a secure home environment. The programme links third sector organisations with Neighbourhood Teams and other agencies to avoid delayed discharges and readmissions for individuals, and to enhance capacity throughout the system.

The summary provides insights into Enhance outcomes, benefits for participants and an overview of how it is benefiting the health community. 

“It definitely does [save time] – we had a number [of patients] that stayed on our caseload for many years simply because the only social aspect they have is through having a nurse or clinical support visiting, it was the only thing in their life – but now they are going to lunches, having outings once a week – some have come off of our caseload, suddenly they can do their own medication they didn’t think they could.”
