The Health Needs and Asset Assessment Grant Program supports Leeds organisations to tackle health inequalities through community-led projects that turn local intelligence into practical action.
This approach builds on recent Health Needs and Asset Assessments (HNAAs) carried out in a number of areas of the city. The purpose of an HNAA is to both add data and intelligence to ongoing work in the area which aims to mitigate the impact of poverty and provide a rationale for resource acquisition and allocation. The intelligence gathered has helped create a strong evidence-base, which can inform targeted interventions to improve the well-being of residents.
Small grants to support local action
The Health Needs and Assets Assessment (HNAA) grant programme will help community groups improve local health and wellbeing. This programme is funded by Public Health and will be distributed by Volition Leeds (on behalf of Forum Central).
Purpose of the Fund
The core purpose of the fund is to provide seedcorn funding for local organisations to improve local health and wellbeing outcomes in line with priorities identified in the recent HNAAs in Gipton, Harehills, Burmantofts and Richmond Hill, Cross Flatts Park (Beeston Hill), Holbeck, and Armley.
Funding Details
Total fund: £63,000 across 13 neighbourhoods in Leeds
Grant amount: Grants of up to £2,000 are available for constituted organisations, and up to £500 for unconstituted community groups.
Applications close: 5.00pm on Friday 5 December 2025
Funding deadline: All funds must be spent within 6 months from receiving the funding
Monitoring deadline: An end-of-project report must be submitted at the end of the project

Organisations based in, or supporting people living in, the following areas are eligible to apply:

We welcome applications from:
We welcome applications from a range of organisations, particularly those working at a very localised level in the areas stated above.
Applicants should:

Be based in Leeds and deliver activity in at least one of the seven focus areas

Have relevant safeguarding, equality, and insurance policies
-
Hold a bank account with two unrelated signatories (or contact us to discuss options)

Eligible Activities
We are looking to fund innovative activities and activities with a track record of positive impact that respond to one or more of the priorities identified within the relevant HNAAs.
Ineligible Activities
Please note the fund will not support:

For-profit or statutory organisations (e.g. schools or hospitals)

Retrospective funding for activities or costs already incurred

Grant-making to other organisations or individuals
-
Activities promoting political or religious viewpoints
-
Capital projects (e.g. building work)

However, applications will be processed as they are received, and may close earlier if all funds are allocated before the deadline.
Grant decisions will be made by a partnership panel comprising representatives from:

A Public Health lead
-
A representative from Leeds City Council/local NHS

A local Third Sector organisation
Successful applicants will be notified and required to provide due diligence documents.
Upon approval, applicants must sign a grant agreement to receive the funds.
Monitoring and Reporting
At the end of the project, successful applicants will be asked to share:
