The HomeFirst Programme is developing and implementing a new model of intermediate care services to achieve more independent and safe outcomes, helping more people to stay at home, whilst improving the experience for people, carers, and staff. It brings together several existing transformation initiatives (including Active Recovery, the System Flow Programme, and Enhanced Care at Home).

Join the programme team on Microsoft Teams to hear more about the HomeFirst programme and ask questions of the team. You can ask questions on the day or you can send them into the HomeFirst inbox before the session by emailing [email protected].
