Issue two of the HomeFirst Highlights is out now, linked below. The HomeFirst Programme team is running an online Q+A session on 12 September, 1pm – 1:45pm.
HomeFirst Q+A Session
The HomeFirst Programme is developing and implementing a new model of intermediate care services to achieve more independent and safe outcomes, helping more people to stay at home, whilst improving the experience for people, carers, and staff. It brings together several existing transformation initiatives (including Active Recovery, the System Flow Programme, and Enhanced Care at Home).
Join the programme team on Microsoft Teams to hear more about the HomeFirst programme and ask questions of the team. You can ask questions on the day or you can send them into the HomeFirst inbox before the session by emailing [email protected].
Sign up for the meeting by contacting [email protected]
The session will be recorded for those who cannot attend. Please share with your colleagues who may be interested to learn more about the HomeFirst Programme.
