On Monday 6th February, Forum Central hosted an introductory session on the Yorkshire & Humber Care Record (YHCR), delivered by Zoe Richardson and Rochelle Featherstone from NHS West Yorkshire Integrated Care Board.

The aim of the session was to inform Third Sector members about the YHCR; what it is and what it does, but also further develop the conversation around Third Sector support for digital integration.

The Yorkshire & Humber care record  is a regional shared care record, which uses a portal called Interweave.

YHCR pulls together health and care information about people collected by organisations which have demonstrated legitimate direct care relationships with people and service users. The YHCR is designed to be a resource for the whole system of health and care including the Third Sector. The YHCR  supports the region’s vision to connect care to improve people’s lives. It has been developed with the intent of enabling all organisations providing direct care to people across the region to share care information with one another.
