This issue provides updates from each of the six programmes: stroke, diabetes, Cardiovascular Disease (CVD) prevention, personalised care, palliative and end of life care and unpaid carers. The theme for the edition is health inequalities with content intended to highlight some of the work being undertaken across WY to improve outcomes for those more at risk of adverse outcomes.
